One form is required for each room and all intended guests for that room.

First section - please provide:

Name (Enter full name) Phone (Enter mobile Number)

Email (Enter your Email Address) Guests (Select number of guests from drop down)

Arrival (Select from drop down) Departure (Select from drop down)

In the "Comments" section below please provide the following:

1. Names of all guests included in your booking.

2. Home address, including state and postcode for each guest.

The sole purpose of this form is for contact tracing and will be kept for a period of 28 days.

As a reminder, we are a completely non smoking environment and do not accept children under 18 or pets.
